So you _are_ working with a book! As both Art and I have said, one chapter per file is the usual and comfortable way to go.

Go with that approach, and the answer to your second question probably becomes "in the .book file, check the numbering properties of File B". Right-click on the File B line in the .book, select Numbering from the context menu and take a look at the Chapter tab
