I tried with a couple very small ditamaps, and was able to get a FM book out of them, so I have increased confidence in the feature. There is nothing special that I know of about this ditamap.

I have not made any changes to the structure apps or anything else.
The failure occurs after I choose the ditamap, but before I name the FM book.

I opened and saved in Notepad. No luck. So I opened and saved in FM. Again no luck.

So I went to 2 online XML file validators and validated the ditamap. No errors were found either time.
